To the editor: Nomura and colleagues (1) recently reported the results of a case control study of the association between gastric carcinoma and serum pepsinogen I levels in Oriental men in Hawaii. Their case and control groups were derived from a population participating in a prospective cohort study, the Honolulu Heart Program. This population, consisting of approximately 7500 largely first- and second-generation Japanese men, was at considerably higher risk of acquiring gastric carcinoma than the U.S. population as a whole. The authors reported annual age-specific incidence rates of gastric carcinoma ranging from 23.1 cases per 100 000 population for ages
