    In 1980 a woman was admitted to the oncology unit of a Denver medical center. She seemed to be in considerable distress. A nurse summoned a medical resident, and the initial interview went as follows.

    Dr. M: "Hello Mrs. L, Fm Dr. M. Can you tell me about your problems? What led you to come to the hospital?"

    Mrs. L: (whispers) "I'm hemorrhaging, Dr.—I've had a nosebleed for 2 weeks."
