    Abstract

Segmental renal vein renin sampling identified a localized high renin source in a patient with a 9-year history of hypertension associated with a segmental renal infarct. The peripheral level of renin was within normal limits, and the initial renal vein renin study was of borderline significance. The patient's blood pressure is substantially reduced 1½ years after nephrectomy, with continued use of less vigorous drug therapy.
