SUMMARY
A presystolic click produced by permanent intracardiac pacemaker catheters is described. Its pitch, timing, and respiratory variation preclude an intracardiac origin. Fluoroscopy shows that it is not due to diaphragmatic movement. It is concluded that the click is produced by pacemaker-induced contraction of the intercostal muscles of the left anterior chest wall.
