    We read with appreciation the innovative article by Kerr and colleagues (1). However, the accompanying editorial by Phillips and Twombly (2) is based solely in the “index condition” framework, in which one condition is viewed to be of paramount interest. The editorialists say that hypertension is the most important condition that is not well managed.
