Published letters are edited and may be shortened; tables and figures are included only selectively. Authors will be notified that the letter has been received. If the letter is selected for publication, the author will be notified about 3 weeks before the publication date. Unpublished letters cannot be returned.

    Annals welcomes electronically submitted letters.

    IN RESPONSE:

I appreciate Dr. Ahmed's comments, and I agree that ACE inhibitors are likely to be beneficial in patients with systolic heart failure and renal insufficiency. The observational studies that demonstrate ACE inhibitor effectiveness in this setting are reassuring (1-3). However, subgroup analyses from randomized clinical trials of ACE inhibitors would add valuable and more definitive evidence to support the use of these agents in these patients.
