If physicians do not decide when and how tests for the detection of human immunodeficiency virus (HIV) are to be used, others will quickly do so. Examples are the compulsory premarital HIV antibody testing required of couples applying for marriage licenses in Illinois and the voter initiatives that have appeared on the California ballots (most recently, Proposition 102). The California initiatives have failed to gain passage twice, partly because of strong campaigns waged against them by the California Medical Association and professional public health organizations.
