    The advent of a new Administration in Washington offers a fresh opportunity for the medical profession to rethink and redefine its approach to long-term care for the victims of severe chronic disease and disability. It is a good time to do so. The current more pragmatic, less ideological political climate provides opportunity for a practical solution to a problem that is as complex as it is urgent.
