We thank Dr. Haywood for his comments and appreciate the opportunity to learn about these related findings. As we mentioned in our paper, we took advantage of the opportunity to conduct secondary analyses on the Beta Blocker Heart Attack Trial because this is one of the few large studies of a clinical event to include adequate data on clinical determinants and outcomes as well as social and psychological predictors. Unfortunately, we were limited to the cohort as assembled. No women were included in the supplemental Health Insurance Plan substudy, from which the data on social class and psychosocial factors were obtained. Future research should include women to determine the generalizability of our findings.
