IN RESPONSE:
We believe that in cirrhotic patients, as clearly shown in Bruno and colleagues' observation, a sustained response to interferon-
2a therapy does not eliminate the risk for hepatocellular carcinoma. We agree that these patients, and all cirrhotic patients, should be carefully monitored for early detection of hepatocellular carcinoma. However, we think that in patients without cirrhosis who have a sustained response, the risk for developing cirrhosis and hepatocellular carcinoma is probably extremely low. Of course, further studies on larger populations with a longer follow-up are needed to assess the long-term outcome of patients after interferon-
therapy.
