Once-Daily Aminoglycoside Dosing

TO THE EDITOR:

Hatala and colleagues' meta-analysis of once daily aminoglycoside dosing [1] should be helpful and encouraging to physicians who are interested in this dosing regimen. However, the use of aminoglycosides has recently diminished with the advent of such agents as broad-spectrum cephalosporins that give adequate gram-negative coverage even for neutropenic patients. The main use of aminoglycosides in my practice is for synergy; the drugs are usually used with a ß-lactam agent for various streptococcal, enterococcal, and staphylococcal infections. In these cases, the drug is given in a lower dosage but usually two or three times per day. I would appreciate the authors' thoughts on the use and monitoring of once-daily dosing for these indications.
