TO THE EDITOR:
I found Halm and colleagues' research interesting [1], but it is remarkable that an article that bases most of its conclusions on echocardiographic ejection fraction that was "assessed visually" was accepted for publication by the Annals. How happy must be the echocardiographers who can, without fail, differentiate visually whether left ventricular ejection fraction is less than or greater than 30%! Ditto for the editors who believe this and do not require more objective data.
