A Library for Internists IX: Recommendations from the American College of Physicians*

The American College of Physicians has compiled its ninth library of reference materials designed to help internists and community libraries acquire and apply current knowledge in internal medicine. The Library for Internists began as a compilation of printed materials, but now that powerful computers and computer networks have become ubiquitous, its character is changing to include electronic information resources that will eventually compose a virtual medical library.

The subcommittee charged with the task of approving this version of the Library faced the same two-pronged dilemma experienced by all students of medicine. First, substantive medical knowledge continues to grow at an increasing pace; remaining current in even a portion of a medical discipline today requires far more knowledge than was needed decades ago. Second, the mechanisms by which medical knowledge is created and disseminated are growing at an equally rapid rate. By the time the "A Library for Internists IX" is disseminated, new Internet-based medical libraries and clinical information will have been introduced by publishers, professional societies, and information systems vendors. In the near future, a new class of intelligent agents, information filters, collaboration tools, and clinical information not even blueprinted will be in use.

Accordingly, the Library's mission is to address both elements of this dilemma. First, we introduce many traditional medical textbooks and reference works that cover the growing subfields of medicine. Many of these works are available in electronic form. Second, we provide a listing of representative software programs, professional organizations, and other resources that will help internists build bridges to these emerging digital medical libraries.


Selection of Resources

The list of books in this edition of the Library was generated by an array of medical information experts and was expanded by recommendations from practicing general internists. College members were the major source of nominations of medical resources. The College Officers, Regents, and Governors and members of the Council of Associates and committees and subcommittees were included in the nomination process. Nominations were also requested from other medical societies and from the editorial boards of selected medical journals.


CD-ROM and Internet-Based Content

In contrast to the state of affairs that existed when the last Library was published (1994), most major textbook publishers are now marketing or developing digital forms of their printed works, primarily on CD-ROM. Some are sold as separate works, some are bundled with the print copy, and some are combined into more extensive CD-ROM libraries. When possible, we have identified books for which a CD-ROM edition is available; however, the reader should assume that most medical publishers will have CD-ROM editions available.

Similarly, full-text medical books and journals are increasingly available from publishers and discount distributors through online or CD-ROM-based subscription. Commercial online service providers offer unlimited access to MEDLINE, many medical journals, and a selection of medical textbooks; medical or hospital libraries also offer such access. Many professional societies make their publications available through their own World Wide Web sites or through consortia that focus on research publications. We only refer to these resources briefly because the ever-changing resource market will make some products obsolete by the time this edition is published.

Pricing of digital resources and online services is in flux. In principle, information servers can maintain logs of reader behaviors and couple reader preferences with selective advertisements and services. Direct charges to the reader can be lower than the true cost of providing a service because revenues are generated through online advertising, theoretically, by coupling reader profiles with targeted advertising and sales materials. Clients of online services should be aware of how information providers generate revenues and who, if anyone, has access to account information and logs of usage patterns.


Gaining Access to Medical Knowledge

The College believes that it can best serve the medical community by promoting awareness of new means of acquiring information. The College's Web site is an early initiative in this area (http://www.acponline.org). Equally vital is the role of the medical librarian. As they begin to add digital media to their personal libraries, physicians would do well to involve specialists from the medical library community.


Use of This List
space

We have changed the organization of this Library to provide several outlets from which to obtain medical information. If one information source becomes outdated, several others will give physicians an idea of where to begin their search some years after a publication date.

With the exception of drug information systems, large-scale databases distributed primarily by CD-ROM or via the Internet are contained in the final section of the Library. We present a few of the myriad online databases, a cross-section of firms that offer access to these databases, and examples of Web sites that are notable for the medical information they provide. Also included are representative services offered by traditional providers of citations and medical literature (services provided by firms traditionally associated with document delivery).

The compilation of this edition of the Library has reinforced an important lesson: The way in which physicians collect medical information is changing dramatically, and the practitioner's need to differentiate between the insignificant and the notable information resource has never been more acute.

Citation and Literature Services

Most vendors of MEDLARS and other citation databases have expanded both the scope and methods of delivery of their products. Many now provide full-text journals in addition to a wide array of relevant clinical databases. Many literature databases are prepared by the National Library of Medicine (http://www.nlm.nih.gov), which makes its data available to the public and to vendors. The charge from the National Library of Medicine is mandated to cover only the cost of distribution and provision of service, not the cost of preparation. Commercial vendors differentiate their products by providing alternative ways of integrating, searching, and viewing data from government and commercial sources. Rather than presenting individual products, we describe a small number of representative vendors. Practitioners should consult their health science librarians before obtaining individual subscriptions. Institutions often receive site licensure at a discount and can therefore offer it to individual persons at a discount.

Databases

Searching a specific database can be satisfying, but the practitioner is often left to decide which database to search. Because MEDLINE, the database most known to physicians, is only one of many available, the services of a reference librarian can be valuable in many circumstances. Listed below are representative databases offered by most of the online service vendors described above.

HealthStar. National Library of Medicine, Bethesda, Maryland, and the American Hospital Association, Chicago, Illinois. HealthStar contains citations to the published literature on health services, technology, administration, and research. It focuses on both the clinical and nonclinical aspects of health care delivery. References to more than 500 English-language publications with special emphasis on economics and legal policy and planning.

MEDLINE. National Library of Medicine, Bethesda, Maryland. The foremost U.S. database in biomedicine from the National Library of Medicine, MEDLINE is one of the world's most vital resource libraries. Because it is available to the public at little expense, MEDLINE databases and access can be purchased from a wide array of online services and CD-ROM vendors.

PDQ. The National Cancer Institute, Bethesda, Maryland. Physician Data Query (PDQ) is the National Cancer Institute's expanding cancer information database. It provides information on the status of management of neoplasms, a summary of active and closed clinical trials, and directories of physicians and organizations concerned with cancer prevention and treatment. Many PDQ documents are written in two versions: one in technical language for health care professionals and the other in laymen's terms for patients and family.

PREMEDLINE. National Library of Medicine, Bethesda, Maryland. This new database provides basic citation information and abstracts before they are put into MEDLINE. It is updated each weekday. Once an article is indexed, it is added to MEDLINE and its PREMEDLINE record is deleted.

PsycINFO Database. American Psychological Association, Washington, D.C. The PsycINFO database covers the professional and academic literature in psychology and related disciplines. Coverage includes references and abstracts to 1) more than 1300 journals in more than 20 languages and 2) book chapters and books in English. More than 50 000 references are added annually, excluding popular literature.

The CINAHL Database. CINAHL Information Systems, Glendale, California. This resource contains references to more than 900 English-language nursing publications plus primary journals in 17 allied health fields and health sciences librarianship. Abstracts and selected full-text available. Available online and from CD-ROM vendors.

Document Delivery Services

Many organizations that previously emphasized document delivery services now provide a more extensive array of online services to the physician. Many originated as subscription delivery agencies to libraries and, as libraries have changed how they acquire information resources, these organizations have expanded to include a wide range of Internet-based services. The Library for Internists Core Committee of the American College of Physicians was chaired by Drs. Mark E. Frisse and Valerie Florance. Dr. Frisse is the Director of the Bernard Becker Medical Library and Associate Dean for Academic Information Management, Washington University School of Medicine (St. Louis, Missouri). Dr. Florance is the Director of the Edward G. Miner Library, University of Rochester Medical Center (Rochester, New York), and is an assistant professor for the School of Nursing and the School of Medicine and Dentistry at the University of Rochester.
