IN RESPONSE:
We read with interest the letter by Papo and colleagues. Although it is difficult to make a generalization on the basis of a brief case report, we hesitate to call their experience a clinical failure because of the apparent low dose of interferon-
used (5 MU three times a week). In our experience, the dose of interferon- necessary for initial control of the hypereosinophilic syndrome has been as high as 8 MU/d. This dose can often subsequently be decreased. Among many patients in our study, we noted a wide dose range that was initially necessary for control of eosinophilia. We emphasize the importance of tailoring each recipient's dose and stress that a daily dose of interferon- may be necessary to control eosinophilia.
