We were surprised to read the letter by Lee and colleagues [1]. We had been requested by the Editors of Annals to respond to their commentary [2] and were provided with only two versionsan original dated 30 October 1993 and a revised draft dated 24 January 1994. Both drafts, provided by the GUSTO investigators to Annals, stated that the rates of coronary artery bypass surgery were 9.5% for accelerated tissue plasminogen activator (tPA) and 8.5% for streptokinase.
