IN RESPONSE:

We found Dr. Saleh's observation and comments interesting. The reduction of chyluria with an ACE inhibitor is indeed a novel observation that will require confirmation by prospective studies. If confirmed, the mechanism could be a hemodynamic effect on the lymphatic circulation, resulting in reduction of chyluria. We are aware of one report claiming that an ACE inhibitor reduced the amount of protein lost across the peritoneal membrane in patients undergoing peritoneal dialysis [1]. A link could exist between these isolated observations.
