Searching for Patent Foramen Ovale in Patients with Stroke

TO THE EDITOR:

In their discussion of cryptogenic stroke, Di Tullio and colleagues (1) fail to mention embolization of pulmonary vein thrombi after pulmonary infarction as a cause of embolic occlusion of a systemic artery. We reported a case (2) in which the patient had unrecognized sustained pulmonary emboli and pulmonary infarctions. She experienced a massive pulmonary embolus that dislodged a pulmonary vein thrombus, which then embolized to a systemic artery. The suggestion that pulmonary venous thrombi be considered in the differential diagnosis of embolic occlusion of systemic arteries was also made by Spain and Moses (3) many years ago.
